What does the evaluation entail?

Parents are asked to complete a questionnaire and to forward relevant records and reports prior to the first visit. At the initial consultation I meet with only the parents. This is done to efficiently convey large amounts of important information and adequately address parental concerns. The child is examined at a separate visit, shortly after the initial consultation. A discussion of recommendations ensues, and follow-up visits are scheduled as needed.

How often does a patient need to be seen?

This varies from case to case. Some families may need to be followed every few months so that I can monitor progress and modify recommendations. Patients who are going through significant behavioral turmoil may need to be monitored every one to two weeks. Others may need to be seen only when the next big decision looms, such as prior to a meeting establishing an Individualized Education Plan.

Appointments


Every effort will be made to provide the earliest convenient time for your visits. I will try my best to honor all time commitments. If you are unable to keep your appointment, please cancel as far in advance as possible.

Payment

You will be billed for your visit and provided with a universal claim form that you can submit to your insurance carrier for reimbursement. It is important that you notify my office of any change in address or telephone number, so that we can provide you with proper documentation.


Letters and Reports

In today's health-care and special-education environments, it is extremely important to craft letters and reports very specifically to achieve desired goals for the patient. These documents will be provided at your request and you will be billed for the time taken to generate the document. All efforts will be made to provide these to you in a timely manner.
